At Creative Care and Connections we want you to be in control over the support you receive. We understand that not one care plan fits all. Care is personalized and delivered in accordance with the Individual Support Plan (ISP). This may include:
- Personal care to meet the physical, psychological, social, and spiritual needs of each supported individual
- Development of Independent Living skill such as hygiene, safety, social skills, expressive and receptive communication, meal prep and more
- Bathing, grooming, dressing, and general hygiene
- Support with housekeeping and laundry services as directed by the ISP
- Meal preparations that follows individual diet and safe food handling guidlines
- Companionship
- Establishing routines
- Escorting individual to doctor appointments, outings, shopping, and community activities
- Motivating Individuals towards self-sufficiency and continued independence
- Encouraging a healthy lifestyle
- Behavioral Support
Direct Support Professionals
Our DSPs are experienced, compassionate, and dedicated to delivering the highest quality of care. They undergo training and background checks to ensure they are qualified to provide the quality support your loved one is entitled to.
Creative Care and Connections encourages bringing on DSPs of your choice, including family and friends, to provide paid supports through the local county developmental disabilities program.
