Creative Care and Connections is a licensed Medicaid agency with the State of Oregon Department of Human Services that coordinates and provides in-home and community based support services to individuals with intellectual and developmental disabilities (I/DD). At Creative Care and Connections, we help to coordinate and manage individualized attendant and relief care services
